At Impact Health Direct Primary Care we can help you achieve your weight loss goals. Our program focuses on long term, sustainable weight loss with intensive guidance in nutrition, physical activity, and lifestyle. Additionally, prescription weight loss medication may be prescribed to aid with weight loss if appropriate. Our program was designed by Dr. Sjogren, who is a Diplomate of the American Board of Obesity Medicine, and enjoys helping patients reach their weight loss goals to improve their health and quality of life.

Our Weight Loss Program is just $125/month* and includes the following:

• Once monthly visits with one of our providers

• Initial comprehensive lab panel and labs every 3 months to track progress. Initial lab panel includes: CBC, Kidney/Liver function, Electrolytes, Cholesterol, Thyroid function, Diabetes/blood sugar/insulin resistance testing, Vitamin B12 level, and Vitamin D level

• Monthly detailed body composition measurements (including body fat percentage) with printouts to track progress

• Monthly B12 injections

*In addition, one time initial enrollment fee of $75 will apply

**Direct Primary Care members will receive a discounted price of $60/month for the weight loss program

Body Composition Analysis

We utilize a medical grade device to measure body composition, including body fat percentage and basal metabolic rate. You will receive a detailed print out with your information and analysis.

• Available to non-members for $30/reading

Personalized Nutrition and Fitness Testing

We offer the Nutrigenomix test in our office. The Nutrigenomix test is a personalized nutrition test which provides DNA-based dietary recommendations. Your healthcare provider will counsel you on eating more or less of certain foods based on your DNA, in order to optimize health and performance. See a sample report here.
