Kara Sjogren, DO

Dr. Sjogren is a board certified family medicine physician in Ennis, Texas. She was born and raised in Texas. She graduated from the University of Texas at Austin in 2009 with a Bachelor of Science in Human Biology and UNTHSC Texas College of Osteopathic Medicine in 2013 with her Doctorate of Osteopathic Medicine. She completed her Family Medicine Residency from 2013-2016 at the Memorial Family Medicine Residency Program in Sugar Land, Texas. She then moved to Kentucky and practiced Family Medicine as well as Weight loss medicine. She and her husband are excited to be back home in Texas and near their families.

She is passionate about spending time with her patients and partnering with them in their care. She decided to open her own clinic where she could practice patient centered medicine the way she envisioned when she went to medical school. In the direct primary care model she has the opportunity to really get to know her patients and to be available to them when they need her.

Dr. Sjogren strives to provide holistic care and treat the whole patient. She is passionate about healthy living and she loves partnering with patients to improve their health and quality of life.

In addition, Dr. Sjogren is an American Board of Obesity Medicine Diplomate and enjoys helping patients achieve their weight loss goals.

Alice Bagby, DNP, FNP-C, RN

Alice Bagby is a Certified Family Nurse Practitioner (FNP-C) and a graduate of Baylor University who holds a Doctor of Nursing Practice degree. Prior to entering graduate school, she garnered a decade of experience as a Registered Nurse (RN) serving in the United States Air Force after graduating with her Bachelor of Nursing Science degree from Samford University.

As an Air Force RN, Alice worked primarily in trauma and intensive care units (ICUs) in Germany, Maryland, California, and Afghanistan. She completed the Air Force’s Nurse Transition Program in 2009 at Landstuhl Regional Medical Center in Landstuhl, Germany, along with the Air Force Critical Care Nursing Fellowship in 2013 at Walter Reed National Military Medical Center in Bethesda, Maryland.

While Alice loved working in the ICU, she realized that her deepest passion as a healthcare provider is to help people achieve their health goals before they may come to need a hospital stay. She became an FNP in order to provide patient-centered care rooted in genuine empathy, open communication, and close collaboration. She aims to become her patients’ healthcare partner, prioritizing their health goals and holistic well-being while elevating their quality of life.


Janie Puente, Medical Assistant

Janie was born and raised in Dallas, Texas. She has 17 years of experience in the medical field. She is bilingual and speaks fluent Spanish. She enjoys knitting and crocheting in her free time. She enjoys helping patients to live happier and healthier lives.
