Ages 2-18 *without adult enrolled: $75/month

Adult Ages 19-64: $95/month

Each additional adult (within immediate family): $85/month

Ages 65+: Services rendered to Medicare patients will be billed to Medicare. Medicare patients will not be charged a monthly membership fee. If you are 65 or over and are not enrolled in Medicare, please call us for more information.

Discounts: 10% discount available for military, veterans, teachers and college students (discounts may not be combined)
