Information for Former Patients of Impact Health Direct Primary Care

We no longer provide primary care or aesthetics services as of November 21st, 2025. Dr. Sjogren now does weight management-only programs (see above for more information on weight management).

Medical Records: Your medical records are confidential, and a copy can be transferred to another doctor or released to you or another person you designate only through your permission. If you would like for us to send your records to your new physician, please sign this authorization form and fax it to 410-237-6747 or email it to info@impacthealthdpc.com. Alternatively, you may have the new doctor’s office fax us their own records request signed by you. They may fax the records request to us at 410-237-6747. It is always best to also notify us by phone or text at 972-846-4800 when you submit a records request as emails and faxes may not always go through or may go to spam. Please allow up to 10 business days for records releases.

The Direct Care model allows us to spend more time with our patients without the red tape of insurance.

Direct Care is a membership based model of care in which the clinic/provider directly contracts with the patient for their care. The patient pays a monthly membership fee for their weight management services with our office. We forgo insurance payments which allows us to eliminate a lot of the administrative burden that currently weighs down traditional insurance-based weight management practices. This allows us to restore the patient-clinician relationship and provide personalized, transparent, and affordable care for our patients.
